Cardinal Giovanni Morone (1509-80) remains one of the most intriguing characters in the history of the sixteenth century Catholic Church - with neither his contemporaries nor subsequent scholars being able to agree on his motivations, theology or his legacy. Appointed Bishop of Modena in 1529 and created Cardinal in 1542 by Pope Paul III, his glittering career appeared to be in ruins following his arrest in 1557 on charges of heresy. Yet, despite spending more than two years imprisoned in Castel Sant' Angelo, he managed to resurrect his career and in 1563 was appointed principal legate to the Council of Trent, whereupon he resolved the difficulties besetting the council, which had brought it to a virtual standstill, and guided it to a successful conclusion. Concentrating largely - but by no means exclusively - upon the period of the pontificate of Pius IV (1559-65) and an evaluation of Morone's role as presiding legate at the Council of Trent, this book tackles a number of issues that have exercised scholars. How does Morone's activity at Trent in 1563 now look in the light of the information available in connection with his processo? What was the result of the wider activity of Morone and the spirituali during Pius' pontificate? How did Morone's career progress after Trent, with regards his actions as a diocesan in the immediate post-conciliar situation and his renewed difficulties in the pontificate of Pius V? Through a re-reading of important archival material and a re-examination of the wealth of recently published primary sources, this study revisits these key questions, and analyses the fluctuating fortunes of Morone's career as bishop, diplomat, heretic and cardinal legate.

This paper exploits the staggered adoption of major concurrent health reforms in countries in Europe and Central Asia after 1990 to estimate their impact on public health expenditure, utilization, and avoidable deaths. While the health systems all derived from the same paradigm under central planning, they have since introduced changes to policies regarding cost-sharing, provider payment, financing, and the rationalization of hospital infrastructure. Social health insurance is predicted to increase this share, although the leads of both social health insurance and primary care fee-for-service suggest endogeneity may be an issue with the outpatient share regressions. Provider payment reforms produce the largest impact on spending, with fee-for-service increasing spending and patient-based payment reducing it. The impact on avoidable deaths is generally negligible, but there is some evidence of improvements due to fee-for-service. Considering the corresponding relative reduction in inpatient admissions and the incentives fee-for-service provides to deliver additional services, perhaps there is an overprovision of services in the primary care setting and an underutilization of more specialized hospital services.

Modern passports and national borders are not only inseparable from the rise of global mobility. They are also tied to the emergence of individuals and nations as the primary sites of global power and identity. McKeown's history links the practices of border control to attempts to control Asian migration around the Pacific in the 1880s. New policies to control mobility had to be justified in the context of contemporary liberal ideas of freedom and mobility, generating such principles as the belief that migration control is a sovereign right of receiving nations and that it should occur at a country's borders. McKeown shows how the enforcement of these border controls required migrants to be extracted from social networks of identity and reconstructed as isolated individuals within centralized filing systems. Methods originally created to exclude Asians from full participation in the "family of civilized nations" are now the norm between all nations and have helped to institutionalize global cultural and economic divisions, such as East/West and First and Third World designations.

While ethnic identities are found to play a key role in politics, not all members of a group toe their group's line and vote for its affiliated party. Why do some voters choose not to vote with their group when doing so can often be advantageous given the norms of ethnic favoritism observed across Africa? According to Afrobarometer data, between 30-52% of voters in Sub-Saharan Africa do not vote for their ethnic groups' party. This book argues that as individuals are less readily identified as members of their ethnic group, they are less likely to be treated as if they are members of that group, which in turn weakens their identification with the group. Individuals who weakly identify with their group are less likely to be influenced by their identity when voting. This approach makes this book the first study to theorize and empirically test the effects of the everyday identity construction process on ethnic salience and in turn on vote choice. To test the theory, the book develops the concept of ethnic distance and measures it empirically. Empirical tests find support for the argument in South Africa, Uganda, and the United States. These cases allow me to test the effect of ethnic distance on several different ethnic dimensions (race, language, and region) in a variety of contexts. As a first step toward matching our scholarly concepts of ethnicity to its complexity in the real world, this study is poised to alter the way we think about ethnicity in politics"--
