As an archaeologist with primary research and training experience in North American arid lands, I have always found the European Stone Age remote and impenetrable. My initial introduction, during a survey course on world prehis tory, established that (for me, at least) it consisted of more cultures, dates, and named tool types than any undergraduate ought to have to remember. I did not know much, but I knew there were better things I could be doing on a Saturday night. In any event, after that I never seriously entertained any notion of pur suing research on Stone Age Europe-that course was enough for me. That's a pity, too, because Paleolithic Europe-especially in the late Pleistocene and early Holocene-was the scene of revolutionary human adaptive change. Iron ically, all of it was amenable to investigation using precisely the same models and analytical tools I ended up spending the better part of two decades applying in the Great Basin of western North America. Back then, of course, few were thinking about the late Paleolithic or Me solithic in such terms. Typology, classification, and chronology were the order of the day, as the text for my undergraduate course reflected. Jochim evidently bridled less than I at the task of mastering these chronotaxonomic mysteries, yet he was keenly aware of their limitations-in particular, their silence on how individual assemblages might be connected as part of larger regional subsis tence-settlement systems.

Migraine is an extremely common condition affecting 28 million Americans. Fully 1 in every 6 adult women suffers from this malady. Patients with migraine are frequently seen in the Emergency Room and Urgent Care Clinic for acute treatment of their headaches. Headache generally accounts for nearly 3 % of all ER visits, resulting in almost 3.5 million visits annually. There currently are no widely used guidelines for the treatment of patients with headache in the ER or UCC, and reducing the number of CT scans remains a key goal for administrators in many ER or urgent care settings. In one study, 35 drugs, alone or in combination, were used to treat migraine. The most common medication class used was narcotics, which were used in 25% of the visits. The next two most commonly used classes were antiemetics and NSAIDs. Only 5% of patients seen for headache received headache-specific medications during their visit. ER and Urgent Care physicians generally have little or no formal training in treating these patients. Consequently, they often feel less comfortable ruling out secondary causes of headaches and using appropriate headache treatment medications and procedures. Since many patients have received narcotics in previous visits, providers may view them as drug seekers. Patients seen in ERs and UCCs for acute treatment of their headaches often report high levels of dissatisfaction. They often end up in the ER or UCC because they don’t have a formal treatment strategy for their severe headaches. Health plans and insurance agencies are often frustrated by the high cost of fragmented care for patients with headaches. Because of the environment that most ER and UCC providers operate in, many unnecessary scans and tests are done because of uncertainty of the diagnosis and unfamiliarity of the patient. Primary care physicians are also frustrated by patients with headaches presenting to them after being seen in the ER or UCC, wanting a refill of their narcotics that they were given. Many patients don’t follow up with their PCPs and simply keep returning to the ER/UCC for their treatment. In short, all involved in the care for patients with headache are frustrated by the current system. The care for these patients is clearly not optimal. It doesn’t have to be that way. There are a wide variety of effective treatment options available, but are underutilized in the acute setting. With these treatment options, patients no longer have to be treated only with parenteral narcotics, only to perpetuate the cycle of suboptimal care described above. This concise handbook covers all aspects of acute headache care, including care of the child and adolescent with acute headaches, treatment of pregnant and breastfeeding women with acute headaches and appropriate evaluation of secondary headaches. In addition, two other novel chapters are included: one on caring for older patients with acute headaches as well as a chapter describing how to ensure a seamless transition of the patient back to a headache interested provider. Eighty years ago the largest genocide ever occurred in Nazi Europe. This began with the mass extermination of patients with neurologic and psychiatric disorders that Hitler's regime considered "useless eaters". The neuropsychiatric profession was systematically "cleansed" beginning in 1933, but racism and eugenics had infiltrated the specialty long before that. With the installation of Nazi-principled neuroscientists, mass forced sterilization was enacted, which transitioned to patient murder by the start of World War II. But the murder of roughly 275,000 patients was not enough. The patients' brains were stored and used in scientific publications both during and long after the war. Also, patients themselves were used for unethical experiments. Relatively few neuroscientists resisted the Nazis, with some success in the occupied countries. Most neuroscientists involved in unethical actions continued their careers unscathed after the war. Few answered for their actions, and few repented. The legacy of such a depraved era in the history of neuroscience and medical ethics is that codes now exist to protect patients and research subjects. But this protection is possibly subject to political extremes and individual neuroscientists can only protect patients and colleagues if they understand the dangers of a utilitarian, unethical, and uncompassionate mindset. Stroke is the most common cause of adult mortality in the United States. Antithrombotic agents form the mainstay of stroke prevention. Aspirin produces a modest reduction in the risk of second stroke and transient ischemic attack (TIA, mini-stroke) and is widely recommended for initial therapy. The thienopyridines (Ticlid) and clopodogrel (Plavix) are alternatives for secondary prevention in patients who do not respond to or cannot take aspirin. They are no more effective than aspirin and have been associated with thrombotic thrombocytopenic purpura. The combination of aspirin and extended-release dipyridamole (Aggrenox) has several mechanisms of action and an additive effect on reducing stroke risk compared with either agent alone. A 2-fold increase in risk reduction and favorable safety profile suggest that the combination can serve as first-line prophylaxis against a second stroke.
